Apply for GIBQ Verification

By clicking "I Attest," I confirm that:

1. I have authority to act for the Vendor named above (the "Vendor") and to bind it to this attestation.
2. I have read this entire application. It is true, accurate, and complete to the best of my knowledge.
3. I prepared this application myself, or it was prepared under my direction, from the Vendor's own records. Every statement in it is the Vendor's own statement, and the Vendor alone is responsible for it.
4. GIBQ Operations LLC may publish the Vendor's certification status, and any later change to that status, at gibq.org.
5. GIBQ may suspend or revoke certification if any information is materially inaccurate or incomplete, or becomes so and is not corrected.
6. The Vendor will notify GIBQ within 30 days if its authorized representative changes, and will promptly report any material change to this information.
7. GIBQ™ and GIBQ Verified™ are trademarks of GIBQ Operations LLC. Certification allows the Vendor to display them only under GIBQ's published rules.
8. Clicking "I Attest" is my electronic signature under the federal E-SIGN Act and New York's Electronic Signatures and Records Act. It has the same effect as a handwritten signature, and I agree to sign electronically. Delaware law governs this attestation.

By submitting, the Vendor also accepts the GIBQ Vendor Verification Agreement, Version 2.1 (gibq.org/agreement/v2.1), including the GIBQ Certification Procedures it incorporates.
